This quest is unlocked and automatically begins right after completing the "Commodore Eighty-Sixed" quest. The time has come to destroy the British blockade of the island.
Edward and Charles Vane put their plan in motion: destroy the British blockade and escape Nassau.

1. Follow Charles Vane
Just follow Vane until you reach the pier where your ship is moored.

2. Take the wheel of the Jackdaw
Board the ship and head to the stern, where the steering wheel is located. Before you can take the wheel, you'll need to talk to Anne first.

3. Check on Anne before leaving
Anne decides to leave Nassau and join the crew of the Jackdaw. Edward has no objections and welcomes her aboard.

4. Rendezvous with the fireship
Take the wheel and simply sail over to the ship you see in front of you.

5. Defend the fireship
The fireship will sail toward the British blockade to blow it up. Sail alongside it, and after a moment, several enemy ships will appear and attack you. The fireship has a relatively high amount of health and, unless you leave it completely unattended, it should reach its target location. The biggest threat is probably the barrels that enemy ships drop in its path - so destroy them whenever possible and as quickly as you can.

There will be quite a few enemy ships, though for the most part they will be smaller vessels that you will be able to destroy relatively quickly. There is a much greater chance that your ship will be destroyed rather than the fireship - so watch your health bar and eliminate enemy ships as best and as fast as you can.

6. Ignite the fireship
When the fireship sails near the blockade, it will come to a stop. You must then shoot it with your cannons to set it on fire. Before you do that, first get rid of all ships that attacked you.

7. Wait for the fireship to hit the blockade
Hint:
The fireship will head toward the blockade, and after a moment, a powerful explosion will occur. Don't sail too close so you don't take unnecessary damage.

8. Escape Nassau's waters
Head toward the position marked by the marker. On the way, you will encounter some British ships - you can destroy them or try to get past / around them, but keep in mind that since there is a storm, you won't be able to reach travel speed.
When you get within about 400 meters of the marker, a cutscene will start. You will manage to escape and end up in Great Inagua.

9. Follow Adéwalé
After the cutscene, you will automatically land in Great Inagua. Walk with Adéwalé to the port. Once you get there, after another - this time shorter cutscene, this quest will end and the next one - Do Not Go Gently... - will be unlocked.
