At present, in order for the quest to appear in your journal, you must go to the Elven Temple, where the Bishop Alexandar is. When you get there, you'll also find Gareth there, they both argue with each other.

Before you leave the ship, talk to Gareth who is on board of the ship. He will inform you about his plans.
Gareth went ashore onto the Nameless Isle, claiming that he was going to scout ahead. Something seems off.
Go to the place where Bishop Alexandar (lvl 16) is. Gareth will also be there.
We came upon a stand-off between Gareth and Alexandar at the Elf Temple. Gareth wants to kill Alexandar.
Convince Gareth that he should give up on revenge.
We managed to talk some sense into Gareth. He will return to the Lady Vengeance and await us there.
If you decided to side with Gareth. You have to kill Bishop Alexandar and other Magisters.
We decided to side with Gareth against Alexandar.
We decided that Gareth's actions were unwarranted, and chose to help Alexandar instead.
Gareth is dead.
Gareth has decided to part ways with us.
We have departed the Nameless Isle. We do not know what came of Gareth.
