You will find the spear inside the Goinnhellir cave, but only after you have completed the main story. You must also have all four adrenaline slots unlocked in the skill tree.

The chest is located in a flooded side corridor. To get there you should shoot an oil jar to blow up one of the walls.

The chest is located in a house on a hill and to open it you will have to get the key that one of the soldiers patrolling the camp has.
