You will receive this quest automatically when you reach the Roots' Passage at the end of the Quest for a panacea quest. You need to find a way to open it.

Siora will tell you that you need the seeds of a particular plant to open the door. She will suggest that you ask Ongos what kind of plant it is.
Return to Ongos and ask him how to open the passage.

You need to get to the other side of Roots' Passage and then get to Credgwen. When you get there, you will witness the tierna harh cadachtas being shot by a soldier.
She will summon a monster that will first kill the soldier and then attack your team.
This boss has a lot of HP and armor. His attacks are very strong so get ready for a tough fight.
After you defeat the guardian, the Bridge Alliance spy will shoot the tierna once more. He'll explain why he did it and then he'll try to kill you too.
Fight him and at some point he will try to give up and will ask for mercy. It will be up to you whether you spare him or decide to kill him.
Approach the tierna who is lying unconscious but still alive on the ground and take her to her village. There she will regain consciousness after a while.
