You will get this after examining the altar in the Ruined Forge of Starfall Past. To reach the altar, you must use the last of the gates as a means to get higher up where the altar is located. When you open the last of the gates then pull the lever located on the other side and quickly step onto the gate before it starts to move up.

To get it examine the altar after lowering, and climbing the pipe to the top level of the Ruined Forge Lava Intake.
