Map Fragments are needed to uncover new regions on your map of the open game world available in the Elden Ring.
Fragments of maps of lands on the surface are always found at the base of stone obelisks with glowing inscriptions. In order to uncover the map of an area, all you have to do is reach the obelisk located in this area, which is sometimes quite a challenge, and pick up the map fragment lying at its base.
In the case of lands underground, there are no longer any obelisks; maps have been placed in corpses or are simply lying on the ground.
Locating an obelisk is made a little easier by the fact that the obelisks' locations are also marked on the uncovered part of the map.

Map: AbyssThis map is located on the base of a stone stele, near the ruins of the church. To reach this place you must first get to the area southeast of the Shadow Keep. To do this, you should use Shadow Keep Ladder #1 and Shadow Keep Ladder #2, after which you will reach the Coffin that will take you to the "Castle Watering Hole" site of grace. Then head south and reach the "Darklight Catacombs" dungeon. After passing it, you'll emerge close to the "Forsaken Graveyard" site of grace from where you'll straightforwardly reach this map fragment.
Map: Gravesite PlainThis map is located on the base of a stone stele, on the side of the road. You will be able to pick up this map shortly after you arrive in Realm of Shadow.
Map: Rauh RuinsThis map is located on the base of a stone stele, on the side of the road. The map is located on the bottom / ground level of the region (Rauh Base) and you can reach it by going through the Cave Passage, the entrance to which is located in Scadu Altus. Going through this cave you will first reach the poison swamp and then the Ancient Ruins Base site of grace. Walking further west you will reach the place where this map is.
Map: Scadu AltusThis map is located on the base of a stone stele, on the side of the road. In order to reach this map fragment, you need to go through Castle Ensis and defeat Rellana, Twin Moon Knight. After that you will be able to open the back door of the castle and exit to Scadu Altus.
Map: Southern ShoreThis map is located on the base of a stone stele, on the side of the road. One way to get to this part of the map is to go through Ellac River Cave. To reach the entrance to this cave you should start near the southern walls of Castle Ensis and make your way down the rocks to the cave entrance. You should eventually reach the Cerulean Coast site of grace. You can also take a slightly circuitous route through the "Dragon's Pit Terminus" site of grace and then "Grand Altar of Dragon Communion" site of grace.
Map: Mohgwyn PalaceYou will find this map fragment on a corpse that is found in the ruins at the base of Mohgwyn Dynasty Mausoleum.
