Heart of BayleDropped by Bayle the Dread. Item Effect: Gain power of Bayle at the Grand Altar of Dragon Communion. You can exchange it for Bayle's Tyranny (Incantation) or Bayle's Flame Lightning (Incantation).
Hole-Laden NecklaceGiven by Count Ymir, High Priest when you talk to him for the first time. Use it at a hallowed ruin to sound a Hanging Bell (Finger Ruins of Rhia).
Igon's Furled FingerGiven by Igon (2nd Location). When facing Bayle the Dread, this summons Drake Warrior Igon from his sign.
Lost Ashes of WarIn the room where you have to fight the jars. Used to duplicate Ashes of War.
Well Depths KeyIn the room you can get to by jumping down from the fabric-covered roof. You can use it to unlock the Well Depths Gate.
Academy Glintstone KeyOn a corpse located on a chandelier hanging in the Church of the Cuckoo. To reach this place, you need walk across the rooftops of the academy's buildings.
Academy ScrollAcademy Scroll can be used to obtain Great Glintstone Shard (Sorcery) and Swift Glintstone Shard (Sorcery).
Carian Inverted StatueYou will receive this after you hand over the Fingerslayer Blade to Ranni. This item will give you access to the reverse version of the Carian Study Hall and thus to Divine Tower of Liurnia.
Chrysalids' MementoItem Effect: Broach wrapped in red velvet. Take it to Heartbroken Maiden / Roderika.
Conspectus ScrollConspectus Scroll can be used to obtain Glintstone Cometshard (Sorcery) and Star Shower (Sorcery).
Dark Moon RingIn the room where you fought Rennala, Queen of the Full Moon, To open this chest you need Discarded Palace Key.
Dectus Medallion (Left)In a treasure chest at the top of the tower. You need both halves of the medallion to operate Grand Lift of Dectus.
Dectus Medallion (Right)It's in the treasure chest located on top of the fort. Join medallions to operate Grand Lift of Dectus.
Drawing-Room KeyYou will receive this key from Lady Tanith if you agree to join Volcano Manor.
Haligtree Secret Medallion (Left)It's on top of the tower. When you get both halves of the medallion then you can use them at the Grand Lift of Rold and travel to the Consecrated Snowfield and the Haligtree.
Haligtree Secret Medallion (Right)Hit Albus, who is disguised as a pot in the Village of the Albinaurics and then talk to him. When you get both halves of the medallion then you can use them at the Grand Lift of Rold and travel to the Consecrated Snowfield and the Haligtree.

Sewing NeedleDrops from Demi-Human Chief in the Coastal Cave. Item Effect: Boc the demi-human's prized sewing needle.
Shabriri GrapeIt will drop from Edgar the Revenger who will invade you here. He will only appear when you complete the quest for Irina of Morne and Castellan Edgar (2nd Location) finds out about his daughter's death (?).
Spirit Calling BellGiven by Renna the Witch when you first speak with her. Item Effect: Bell that summons various spirits from ashen items.
Tailoring ToolsDrops from Demi-Human Chief in the Coastal Cave. Item Effect: Enables armor adjustment at site of grace.
Unalloyed Gold NeedleIf you assist Millicent and then come back a little later (or reload) she will be dead and in her corpse you will find the needle.
Valkyrie's ProsthesisIn a treasure chest. Item Effect: Golden prosthesis used by the one-armed Valkyrie. Take it to Millicent (3rd Location).
Weathered DaggerYou will receive it from Fia, Deathbed Companion at some stage during the game.
Mending Rune of the Death-PrinceIt grants access to an alternative ending of the game after defeating the final boss. Item Effect: Can be used to mend the shattered Elden Ring.
Miniature RanniYou must talk to it three times using the site of grace and then the Baleful Shadow will spawn.
