You have to uncover and kill: Skylax the Fair, Monger, Lagos the Archon, Kallias, Silanos of Paros, and Pausanias.
A clue (Donation note) that will help you find this Cultist is located in the National Treasure Chest in the Xerxes Military Fort in Lokris.
Skylax the Fair can be found in his camp on Euboea island.
Items: Spartan War Hero Armor (Legendary Torso, Spartan War Hero Set), Peloponnesian symbol on Skylax's breastplate (Cultist Clue)
Monger is in Korinthia and you will face him in one of the main story / odyssey quests - Monger Down.
Items: Spartan War Hero Helmet (Legendrary Head, Spartan War Hero Set), Sage Letter to the Monger (Cultist Clue), Monger's Men (Epic Crew Theme)
You will receive the clue (Cultist Attack on Naxos) when you complete the main story / odyssey quest - Death and Disorder, and you will face Silanos of Paros (He is on the ship) in Unified Front quest.
Rewards: Spartan War Hero Gauntlets (Legendary Arms, Spartan War Hero Set), Olympic Declaration (Cultist Clue).
You will face Lagos the Archon in main story / odyssey quest Judge, Jury, Executioner.
