Location: Novigrad
You will receive this quest while doing the "Get Junior" quest, when you leave the chess club after the meeting with Radovid.

Quest stages of An Eye for an Eye
1. Meet Roche at his camp.
Go to the Temerian Partisan Hideout and talk to Roche. He will tell you that Ves disobeyed his orders and decided to attack the Nilfgaardian troops in the village of Mulbrydale on her own. He will ask you to help him find her before something bad happens.
Those under Roche's command rarely defied orders. When they did, the task of disciplining them fell to Roche's second-in-command, Ves. No one who endured a reprimand from this tight-lipped yet strong-armed soldier would ever think of insubordination again.
Yet this time Ves strayed from her usual loyalty and ignored Roche's orders by leaving her post without permission. Faced with an unusual dilemma, Vernon decided to turn to the witcher for help.
2. Meet Roche by Hanged Man's Tree.
Roche will be waiting for you near the Hanged Man's Tree signpost. Go there. When you start wondering where Ves may be and when she will attack, Geralt will notice people sneaking into the village. The attack has just begun.
3. Kill all the Nilfgaardians. / Save Ves.
Go to the village. You'll find several Nilfgaardian soldiers (11,12) there. When you get to the village, Ves' life bar will appear on your screen. When it drops to zero, Ves will die so don't let it happen. Ves fights a Nilfgaardian officer in the middle of the village, near the signpost. She may be killed pretty fast, so you should get there as soon as possible.
When you kill the Nilfgaardian soldiers and Ves survive, you'll notice that one of the soldiers is still alive. Ves will want to finish him and Roche will want to spare him. It will be up to you to decide what the fate of that soldier will be.
If you decide that the soldier should die:
The peasants of Mulbrydale had helped Roche's guerillas in the past, so Ves decided to stand in their defense now, in their hour of need, though she knew this was a hopeless and nearly suicidal mission. Had not Geralt and Roche come to the rescue in the last minute, Ves would surely have died a premature death after being cut down by Nilfgaardians. Yet as it were, the imperial troops were defeated - and their leader was killed on the witcher's request.
If you decided that you should spare the soldier.
Yet as it were, the imperial troops were defeated - but their leader escaped with his life thanks to Roche's intervention and Geralt's support.
If you don't manage to save Ves, the quest will end in failure.
Yet Roche and Geralt did not come to the rescue in time. Ves fought to save the peasants of Mulbrydale - though she knew full well she stood no chance of winning against her substantially more numerous opponents. No historian will write of this struggle, nor will any bard sing of it, and no gravestone shall mark Ves' resting place.
After completing An Eye for an Eye you will get or will be able to get Reason of State (1 of 4).
