This main quest starts automatically when dusk falls on the first day, or when you complete the "Like Father, Like Son" quest. When night falls, you will automatically head to the church, and a cutscene will trigger. Depending on whether and how you completed the "Blasphemy" quest, either a banner made by Gremla will hang above the altar while she sits in a pew - or her corpse will be hanging there instead.

Blood Mass is tonight - and Knyaz Brencis and his retinue will be at the church to oversee the collection of tithes. We're worried about Mum. Vrakhiri don't look kindly on the weak.
1. (Optional) Pass time until sunset / Go back to Pieter to wait till the Mass
There's still some time until the Mass. I'm free to spend it as I see fit. I could spent the rest of the day with Dad and we'd go to the Mass together.
This objective will appear if you completed the "Like Father, Like Son" quest early enough and still have some free time left on the first day. It is best to spend this time doing side quests, but you can also talk to your father to advance time and bring on the evening.
2. (Optional) Talk with other villagers
The whole village is here. A good opportunity to catch up with neighbours.
If you want, you can talk to other villagers who have gathered in the church. To complete this objective, talk to all the named residents of your village.
3. Sit in the pew next to Pieter and Esme
The Mass begins soon, I should take a seat.
Simply sit in a bench next to your parents. Brencis and his henchmen will arrive at the church - and the Mass will begin.
Depending on whether you managed to properly prepare the strengthening potion for your mother and give it to her (the "Withering Away" main quest), she will either survive the Mass or die.
Either way, after the Mass, your family will return home - and your father will decide it is time to join and start the rebellion. This quest will end, and the next one will begin - Bad Blood. Reward: 250XP
4. Endings
If you correctly prepared the potion for your mother.
Thanks to Anca's herbs, Mum made it through this mass. But Brencis made it clear that he won't tolerate such weakness among his subjects. What do we do now?
If you fail to deliver the herbs on time or if you make a mistake while preparing the potion.
