Fast travel to Barvechos Charlie and go to Barvechos City.
Expect several road blockades on your way, Unidad helicopter, etc.
Go to Charamokho Village.
You need to follow the van. To be able to receive data you must have visual contact (line of sight) and be closer than 160 meters, but not too close. The biggest problem is enemy cars passing by. You can go off the road and try to avoid them or stop and kill them (they can detect you but not too close to the van).
