To start this quest you should talk to a healer named Gregorius Swann. You will find him next to his house located northeast of the Stonegarden Cemetery.

Quest stages of A Danger to Herself and Others

1. Path #1 (Recommended): Healer Gave Quest

If persuasion succeeds ([Intelligence] check or SCHOLAR) then he will give you the Key to open his house and the hatch To Healer's House Cellar.

Swann told us about a sick young woman whom he's tending to in his cellar. We convinced him to let us see her for ourselves, but only if he accompanies us.

1.1. Enter Cellar

We have entered the cellar, which appears to be used as an infirmary. The woman should be close.

1.2. Operation Successful

You need a character with Scholar talent to be able to cure Natalie Bromhead. Approach her with that charater then select ...

[SCHOLAR] *Point out a partially healed circular wound upon the woman's scalp - a trepanation wound. Someone opened her head and tinkered around, it seems.*

... then ...

*Agree - if there's even a small chance of saving her, you have to try.*

Four additional enemies will spawn: Natalie's Anguish, Natalie's Despair, Natalie's Fury and Natalie's Fear - kill them (for XP) and then weaken Natalie (<50% HP). Be careful! Don't kill her!

Note: Additional monsters will disappear when you weaken Natalia, so you can skip them, but then you will not get experience points for killing them.

Operation Successful

Wait for Swan to come to Natalie's body then select ...

*Begin the operation.*

... then ...

*Ask Swann to help keep her steady.*

... then ...

*Select a pair of delicate tweezers and gently try to remove the maggot.*

... then ...

*Slowly close up the wound.*

The result will be ...

We successfully operated on the young woman. It looks like she will make a full recovery. We should inform Swann of our success.

1.2.1. Told Healer Of Life

Talk to Natalie Bromhead then talk to Gregorius Swann.

We told Swann that the young woman is well now. He was astounded by our success, and agreed to help her through the rest of her recovery. He will also be available to us as a healer and trader from now on.

1.3. Killed Daughter Healer

If you do not have a Scholar in your team, or if you hit Natalie too hard and kill her or you could not cure her because you chose the wrong options, then you have to kill her.

We were forced to kill the afflicted young woman after she attacked us. Swann has vowed to study her remains, in order to figure out what happened to her.

2. Path #2: Healer Declined

If persuasion fails.

Swann told us about a sick young woman whom he's tending to. He refuses to let us see her for ourselves, however.

2.1. Enter Cellar

Use the back entrance to the house to get inside unnoticed. The door to the house is locked so you have to lockpick it.

Unlock the hatch To Healer's House Cellar and go down to the cellar.

We have entered the cellar, which appears to be used as an infirmary. The woman should be close.

2.2. Killed Daughter

Come to Natalie Bromhead. Four additional enemies will spawn: Natalie's Anguish, Natalie's Despair, Natalie's Fury and Natalie's Fear - kill them all, including Natalie.

The afflicted young woman is dead.

2.2.1. Told Healer Of Death

We told Swann that the young woman in his cellar is dead. He was not pleased.

3. Healer Died

Gregorius Swann is dead.

4. Close Left RC

We left Reaper's Coast without fully investigating Gregorius Swann's problem.
